CASE MANAGER
A vacancy exists within Council’s Aged and Disability team for a person to provide high quality and responsive case management and assessment.
Responsibilities include assessing clients’ and carers’ needs, developing and monitoring of individual service / care plans in consultation with clients, families and service providers (including GPs) and organising a range of services to meet identified needs.
Applicants will need to possess the following attributes:
· Excellent communication skills,
· Knowledge and understanding of health and community service systems,
· Commitment to the philosophies of the HACC program,
· Access to existing networks within the region and resourcing those outside the area,
· Capability to creatively meet the needs of clients and others involved with service planning and delivery,
· Experience in working with people who are aged, frail, have a disability or dementia and their families,
· Acceptable qualifications within the Health or Community Services field,
· Willingness to travel throughout the municipality including servicing clients in Buloke Shire.

Salary: Attracting $44,173 per annum the appointment is full time and shall initially be for 2 years to January 2010.
