CASE MANAGER / ASSESSMENT OFFICER
PART TIME POSITION
A vacancy exists within Council's Aged and Disability team for a person to provide high quality and responsive case management and assessment.
Responsibilities include assessing clients' and carers' needs, developing and monitoring of individual service / care plans in consultation with clients, families and service providers (including GPs) and organising a range of services to meet identified needs.
Applicants will need to possess the following attributes:
· Appreciation and understanding of key concepts of Case Management, Assessment and Care Planning and service delivery in a rural community setting.
· Knowledge and understanding of health and community service systems,
· Commitment to the philosophies of the HACC program.
· Access to existing networks within the region and resourcing those outside the area.
· Capability to creatively meet the needs of clients and others involved with service planning and delivery.
· Experience in working with people who are aged, frail, have a disability or dementia and their families.
· Excellent communication skills.
· Acceptable qualifications within the Health or Community Services field.
· Willingness to travel throughout the municipality including servicing clients in Buloke Shire.

Salary: Attracting approximately $24.00 per hour this is a temporary part time position at 2 days per week (flexibility available) offered to December 2011.
