Swan Hill Rural City Council, through Aged and Disability Services, aims to support, enhance and monitor the wellbeing and independence of frail, aged, people with disabilities and their carers.

Council provide a flexible service which responds to the varying needs of clients and carers.

Funding is provided through the Aged and Disability Program - Home and Community Care (HACC) and other State and Federal Funding.

Services include:

•   Package Care

•   Food Services (Meals on Wheels)

•   General Home Care

•  Personal Care

•   Respite

•   Home Maintenance

•   Senior Citizens

•   Social Support & Planned activities

•   Veterans Home Care

 Veterans' Home Care
Veterans’ Home Care provides a range of services for Veterans and Veteran Widows
 Social Support and Planned Activities
Provides for social interaction for clients who have little community access.
 Respite Care
For carers of dependant frail aged or a child or adult with a disability to enable the carer to have a break.
 Personal Care
This service is provided to assist with showering, bathing, dressing, etc.
 Home Modification and Maintenance
An appointment is required to access this service. Provides assistance to maintain a safe secure home environment.
 Food Services (Meals on Wheels)
This service is directed towards the frail, aged and people with disabilities and their carers, who are assessed as being nutrionally at risk.
 Package Care
Package Care provide Case Management to help people with complex care needs
 General Home Care Services
This service is directed towards the frail, aged and people with disabilities and their carers to maintain a safe, secure, healthy home environment.
