Community Care

Swan Hill Rural City Council, through Community Care Services, aims to support, enhance and monitor the wellbeing and independence of the frail, aged, people with disabilities and their carers.

Council provides a flexible service which responds to the varying needs of clients and carers.

Programs are supported by funding from the Victorian and Commonwealth Governments.

 Senior Citizens Centres

 Veterans' home care
Veterans’ Home Care provides a range of services for Veterans and Veteran Widows
 Social support and planned activities
Provides for social interaction for clients who have little community access.
 Respite care
For carers of dependant frail aged or a child or adult with a disability to enable the carer to have a break.
 Personal care
This service is provided to assist with showering, bathing, dressing, etc.
 Property maintenance
Provides assistance to maintain a safe secure home environment.An appointment is required to access this service.
 Delivered meals (Meals on Wheels)
This service is directed towards the frail, aged and people with disabilities and their carers, who are assessed as being nutrionally at risk.
 Home and Community Care (HACC) -Package care
Package Care provide case management and packages to help people with complex care needs to maintain indepencence and remain living at home
 Home and Community Care (HACC) Services - Domestic assistance
Home and Community Care(HACC)funded Domestic Assistance has Aged Care Packages that respond to the varying needs of clients and careers
