Eligibility

This service is directed towards the frail, aged and people with disabilities and their carers.  To determine whether a person is eligible, a variety of questions relating to individuals circumstances, including health are asked.

People wishing to use the service, their family or friends may apply.  Referrals are also received from other services such as District Nursing.  It is not necessary to have a medical referral.

Comprehensive Assessments are conducted by appropriately trained staff, who are sensitive to the needs of persons requiring assistance to determine a person's needs and eligible.  When you request assistance an appointment will be made for an Assessment Officer to visit you in your home.  They will provide information, to assist you in making a choice from the options available.  In some instances different people involved in your care may be consulted to clarify needs and/or health services.

Fees

This program is flexible in order to meet the varying financial needs and requirements of the client.

The Assessment Officer will discuss the cost with you, during the assessment.
