Swan Hill Rural City Council, through Community Care Services, aims to support, enhance and monitor the wellbeing and independence of frail, aged, people with disabilities and their carers.
Council provides a flexible service which responds to the varying needs of clients and carers.
Programs are supported by funding from the Victorian and Commonwealth Governments.
Services include:
