To request a copy of an immunisation history statement for immunisations given after 1996, visit the Australian Immunisation Register.
Before 1996, vaccination records were kept by the person or service that gave the vaccination, such as your local council immunisation service or GP. Your vaccination details may also have been written in a baby health book for your parent or carer to keep at home.
We can search our Council records to see if we hold any of your immunisation records from before 1996.
If your immunisation was given after 1996, visit the Australian Immunisation Register to get your immunisation history.
If your immunisation was given before 1996, go to the next step.
You will need to provide your (or your child's) name, date of birth, address and medicare card details.
